The Distal Shoe appliance is used when a second deciduous molar is lost prematurely and the first permanent molars are erupting mesially. The appliance generally uses the first deciduous molar for anchorage and a loop is placed in the eruption path of the first molar. A subgingival spur is then added for guidance.
This appliance should be cleaned every day with toothpaste and a soft toothbrush, morning and night. Encourage the patient to give sufficient time to clean thoroughly around the appliance, gums, teeth, and bands to maintain proper hygiene and prevent inflammation. 3 minutes is recommended.
